Applying for Psychiatric Care

If you are interested in applying for care, please fill out our contact form. We look forward to meeting you and helping you work toward your recovery. 

If we are accepting new patients, we will send two screening forms where we can learn a little about you and see if we might be a good fit to serve your mental health needs.

Upon acceptance, Samadhi Psychiatric commits to conduct a thorough mental health or psychiatric evaluation. The results will be communicated to you upon completion, and treatment options and/or triage opportunities will be discussed. If an option appeals to you, then your provider will work in partnership with you to create a treatment plan that fits your needs.
